The Cardiology Department of Kunhua Hospital, Yunnan Province's First People's Hospital, successfully completed a high-risk PCI revascularization surgery using the fully magnetic levitation extracorporeal ventricular assist device (Extra-Ventricular Assist Device, MoyoAssist®). The use of this device provided effective hemodynamic support throughout the intervention, contributing to procedural stability and improved postoperative recovery in a critically ill cardiac patient.
Patient Profile
Age: 38
Gender: Male
Medical History: Recurrent shortness of breath and chest tightness for 9 months
Echocardiogram Results:
Left atrial and ventricular enlargement
Reduced left ventricular wall motion
LVEF of 36%
Coronary Angiography:
Severe coronary artery disease:
Complete occlusion of mid-right coronary artery
Near-total occlusion of proximal left anterior descending artery
95% stenosis in the middle circumflex artery
Multiple underlying conditions, making the patient's condition highly critical
Family declined coronary artery bypass surgery
Surgical Challenges
Severe Heart Failure: LVEF of 36%, compounded by multiple underlying conditions
High-Risk Coronary Lesions: Complex coronary artery disease with extensive occlusions
Surgical Risks: High risk of myocardial ischemia and complications during prolonged procedures
Decision: To perform PCI under MoyoAssist® support to stabilize circulatory support and minimize risk
Surgical Procedure
Placement of MoyoAssist®:
Femoral vein puncture for placement of 20F venous drainage catheter into left superior pulmonary vein
Femoral artery puncture for placement of 16F arterial perfusion catheter and distal perfusion catheter
Activation of MoyoAssist® with a flow rate of 2 L/min to stabilize hemodynamics and ensure lower limb blood supply
Percutaneous Coronary Intervention (PCI):
Radial artery approach used to open left anterior descending artery and circumflex artery
Intravascular ultrasound (IVUS) guidance for precise stent placement (5 stents and 1 drug-eluting balloon)
Successful revascularization with TIMI 3 flow restoration
Device Operation & Monitoring:
MoyoAssist® operated steadily at 2500 RPM, flow rate of 1.97 L/min, ensuring stable vital signs during surgery
Post-surgery, the patient was transferred to the CCU with stable hemodynamics, and the device was removed after 1 day
Post-Operative Recovery:
Day 1: Hemodynamic stability and successful device removal
Day 4: Patient able to walk
Full recovery and discharge after a short hospitalization
Ultrasound confirms the catheter placement into the left superior pulmonary |

Clinical Insights
Patient Population: With the aging population and increasing complexity of coronary artery disease, particularly in elderly patients with multiple comorbidities, traditional surgical procedures pose high risks.
PCI Risks: In such cases, PCI is associated with significant risks, including myocardial ischemia and complications.
Device Benefit: The use of MoyoAssist® provides stable circulatory support during high-risk coronary procedures, reducing surgical risk and improving patient outcomes.
